How do I print Form 1095-C Employee?

Print Form 1095-C for any employee of an "Applicable Large Employers (ALE) member who is a full-time employee for one or more months of the calendar. ALE members must report that information for all twelve months of the calendar year for each employee" (irs.gov).

 

Before you get started

  • Convert year-end ACA information for the tax year.

  • Check the form alignment to make sure the information prints correctly on the form. See the help topic titled How do I check form alignment?

 

Printing Form 1095-C Employee

1. Open Connect Payroll > Government Reporting > 1094/1095s.

2. Use the Title menu to select 1094/1095s - 1095-C and 1094-C [Caselle Master].

3. Load Form 1095-C in the printer.

4. Click Print (CTRL+P).

Form 1095-C Employee prints.
