License/Renewal Forms

Use the renewal form to print a renewal notice that will tell the owner that it is time to renew an animal's license.


Before you get started

  • Before you run this report, you need to set up a license form and a renewal form in Organization > Forms.


Printing a license form

1. Open Connect Animal License > Licenses > License/Renewal Forms.

2. Select a report title.

3. Do you want to use the current date as the mailing date? The system will use the Mailing Date as the transaction date.

  • Yes, I want to use the current date as the mailing date. Continue to the next step. The report uses the current date as the default date.

  • No, I want to use a different date as the mailing date. You can click the Calendar button, located next to the Report Dates field, or click Advanced Options.

4. Skip the Due Date field. You won't need it to print the license form.

5. How many license forms do you want to print?

  • I want to print a license form for each owner. Select the One Form per Owner option. The report will print a single license form for each owner even if the owner has more than one animal.

  • I want to print a license form for each license. Select the One Form per License option. The report will print a single form for each license. If an owner has multiple licenses, the report will print a form for each license, which means in this case the owner will receive multiple license forms.

6. Which form do you want to use to print the license form?

  • I want to use the current license form. Click on the Forms tab > Form Options subtab. The form showing in the Form field is the current form. Continue to the next step.

  • I want to edit the current license form. Click on the Forms tab > Form Options subtab. Click the Edit button, located at the End of the Form field.

  • I want to use a different license form. Click on the Forms tab > Form Options subtab. Click on the Form field and then select a new license form.

7. Click Print (CTRL+P).

Printing a renewal form

1. Open Connect Animal License > Licenses > License/Renewal Forms.

2. Select a report title.

3. Enter the Report Date.

The system will use the Report Date as the transaction date.

Do you want to use the current date as the mailing date?

  • Yes, I want to use the current date as the mailing date. Continue to the next step. The report uses the current date as the default date.

  • No, I want to use a different date as the mailing date. You can click the Calendar button, located next to the Report Dates field, or click Advanced Options.

4. Enter the Due Date.

The due date is the date when payment is owed.  

When does the owner need to return the renewal form?

  • I want to use the current date as the due date. Continue to the next step. The current date is the default date.

  • I want to use a different date as the due date. You can enter the MM/DD/YYYY in the Due Date field, click the Calendar button to choose a date, or click Advanced Options.

5. Set up the One Form per Owner checkbox.

Use this option to determine the number of forms to print for each owner.

How many renewal forms do you want to print?

  • I want to print a renewal form for each owner. Select the One Form per Owner option. The report will print a single renewal form for each owner even if the owner has more than one license.

  • I want to print a renewal form for each license. Select the One Form per License option. The report will print a single form for each license. If an owner has multiple licenses, the report will print a form for each license, which means in this case the owner will receive multiple renewal forms.

6. Select the Form.

A form is any document that has been set up to print information, such as text, titles, or variables, in a specific format to plain paper or pre-printed stock.

Which form do you want to use to print the renewal form?

  • I want to use the current renewal form. Click on the Forms tab > Form Options subtab. The form showing in the Form field is the current form. Continue to the next step.

  • I want to edit the current renewal form. Click on the Forms tab > Form Options subtab. Click the Edit button, located at the End of the Form field.

  • I want to use a different renewal form. Click on the Forms tab > Form Options subtab. Click on the Form field and then select a new renewal form.

7. Click Print (CTRL+P).
